I am a UK Taxpayer and would like Hawks Football Club to reclaim the tax on any donations I have made in the last year and any future donations I make.

I confirm I have set up a Standing order for £15 per month to cover annual membership fees of £180 with a reference added to my child's name. Payments are to be made to Hawks FC : Barclays , Bedford. Account : 00605263 Sort Code : 20-05-74 .

Youth Players in our 6 a side Team pay a separate subscription to join to play set at £25 per League season , Adults are £50 per League season.

If payments fall behind and are 3 months in arrears I understand the club will cancel my membership.

I will abide by the club rules and codes of conduct which are outlined in the Hawks FC Members Handbook and Club Constitution which are on the Hawks FC App and Hawks FC Website. I wish to register for the Hawks FC Tournament. I will abide by the Tournament Rules, Ground rules and Codes of conduct which will be advised. I understand that this is a Respect & Fair Play Tournament and Hawks are encouraging that all players attending are offered a minimum 50% playing time over the event.

Hawks FC covers the cost of course fees for our volunteers to be trained and remain qualified. Volunteer positions are commitments and I understand and confirm that if I leave my role within 12 months I will be required to refund Hawks FC in full for the cost of any training courses I have attended which the club has paid for. All club kit provided to me will also need to be returned in full in good condition or I may be invoiced for replacements.
